Illness & Emergency Procedures
Parents or guardians are notified in case of serious illness or accident. The parents will be asked to come for the child in the event of a serious accident or illness where further medical attention is necessary. It is imperative that the school has current phone numbers and emergency contacts on file. Please advise your school of any phone number changes immediately.
A physician’s statement concerning any serious ailment (asthma, diabetes, epilepsy, etc.) should be filed with the school health record. When a child returns to school after a serious illness, it is the responsibility of both the parent and physician to establish the child’s physical limitations with the school.
First aid is the immediate temporary care given in case of accident or sudden illness. If it is not a life-threatening situation and further medical attention is deemed necessary, movement of the child will be directed by the parent/guardian. In life-threatening emergencies, students will be transported by ambulance to a local hospital, and a school employee will remain with the student until a guardian arrives.
Communicable Diseases & Exclusions
District 92 follows the IDPH Communicable Disease School Nurse Guidance. The district balances student rights with the obligation to protect the health of all students and staff.

Medication Policy
Procedures and permission forms are available in the Nurse’s Offices. No student will be permitted to possess or self-administer medication until the proper forms are received. Proper forms are required for students to possess epinephrine auto-injectors or asthma medication for immediate use.
Please update the school nurse immediately if there is any change in your student's medication or medical condition.
Required Examinations & Screenings
All incoming Kindergarten and 6th grade students must have a physical exam on the State of Illinois form, dated within one year of the first day of school. Records must be submitted prior to October 15th. Transfer students must present documentation prior to registration.
Eye Exams: Required for all students new to Illinois. Proof must be submitted by October 15th.
